• You must submit your online application at https://emrs.ehawaii.gov prior to booking an appointment. Appointments submitted with insufficient or incorrect information may be canceled without notice.
• Check your email for your receipt from emrs.ehawaii.gov for your 8-digit Locator ID/Authorization Code.
• Same-day appointments cannot be scheduled online. Call for availability at (808) 586-4544.
